Entry Fee is $125 per person. (You may deduct using any of the following deductions that apply.)

Registration Rules:

Tournament and application rules may change at anytime without prior notice required.
Applications are not transferrable to another individual under any circumstances at any time.
Last years golfers will be given priority of placement involving the 100 player limit (as this years players will be given priority next year too.)
If YOU cancel for ANY REASON or you are late for play, your entry fee will not be refunded and will be reclassified as a donation to a 501c3 Not-for-profit organization (LYSA.)
Only one professional per foursome unless more that 25 register in the first 100 players.
$35 returned check charge (whether playing or not.) A NSF returned check will take you out of the reservation line and put back in once all monies for payment and returned check charges are paid.
If you wish to golf with your own foursome then your forms and payments must all be mailed in the same envelope. If forms and payment of friends in your foursome is not included you may or may not be playing with them in the tournament and you will not be notified until tournament shotgun start.
